Open Google Forms
1. Get to Google Forms the following ways:
Begin Adding Text -see illustration below
1. Create a Title for your form. Let’s use: Getting to Know you at Work.
2. Now, create a title of your form or your document for saving purposes. I would name it the same thing.
3. Create a form description: Complete this form so that every employee of our company can get to know you better!
4. In the Untitled Question area, type your first question/statement: First Name
5. Click Required - this means the user can’t skip the question.
Google Forms is an application in which one can gather information quickly by the user filling out a simple form.

Your form should now look like this:
Notice when you type “Name” as the Untitled Question, that Google Forms intuitively knows what type of question is could be. It automatically filled in the question type as Short Answer. Let’s take a closer look at the question types on the following page.

There are 9 question types to choose from in Google Forms.

Add the following questions to your Google Form by clicking the + button on the right:
Don’t forget to click Required after each question so that the user must answer.
