Surveys -
*As a district, have you given surveys? As a parent or staff member, have you taken surveys?
*Survey question bank - Lists questions, responses, typical audience, time of year to use the question, etc.
*Pre-made Google forms that you can make a copy of, adjust and use
It might be a good idea to send out another survey to families and staff especially during a transition. What additional supports do parents need? How are the different modes of learning working for families as well as staff? What information will your teachers want to see
7 Critical Areas -
*Good, concise handout that might be helpful to use with a school board, parents or staff to keep everyone on the same page about need areas to be addressed. This includes a checklist of areas that might be overlooked. Use this to make sure that all learners/groups are being included in plans.
Monitoring Tool -
*Instructions, as well as a tab for documenting each critical area: Step 1. Determine teams Step 2. Determine data to monitor Step 3. Develop, Enter & Finalize Work Plans Step 4. Update Progress - document evidence
*Very much a personal preference if this tool will work well for your district/building or not

As Lisa said earlier, this tool might not work for every district, but it provides a visual checklist of what needs to be done and who is to complete each task. If this doesn’t work for your district, maybe you could use the format to create a tool that would work for you...it gives you a starting point.
