What is an Accident?

  • An accident is an unplanned, and unwanted event, resulting in mishap including injury, property, as well as fiscal loss.
  • Accidents usually occur due to;

Preparations for an Accident Investigation

  • Develop and present an accident investigation policy;
  • Develop and implement an accident investigation procedure;
  • Organize an accident investigation team with relevant skills, training, experience and resources;
  • Routinely audit the incident investigation policy and procedure to ensure;
    • Workers are aware of their roles and responsibilities
    • System is implementing previous suggested controls
  • It must be ensured, the employee assigned the investigation responsibility is trained, experienced, has knowledge of the work activity.
  • During accident investigation previous records should be consulted as well;

Conducting an Accident Investigation

  • Visit the site for evidences collection;
  • Interview the primary witness (victim);
  • If primary victim is not available, interview the secondary witness (nearby worker) present on the worksite;
  • Check and view the CCTV/ Area Monitoring Camera Videos;
  • Review the evidences, and previous maintenance reports of the involved equipment;
  • Review the work procedures and method statements for any kind of deficiency;
  • Document all important notes before making any change in the worksite and work procedure;
