Transition from CMMI ‘ANCHOR’ Flowsheet and Questions to Epic Foundation SDOH and IPV Questions
What is changing?
The CMMI Flowsheet and Questionnaire that have SDOH and Domestic Violence questions will be retired on 5/10/22
It will be replaced with the NYC Social Factors questions about SDOH (food insecurity, housing quality/stability, transportation) and the HARK tool for domestic violence (screenshots in subsequent slides)

What is staying the same?
FREQUENCY
The questions will be auto-assigned to patients at the same frequency as the CMMI is currently assigned
Medicine: Annually
Pediatrics: Every visit from age 0-6 then annually until age 12
OBGYN: Initial prenatal visit, once from 24-40 weeks, Postpartum Visit
Family Planning patients will receive questionnaires at these same frequencies (same as current CMMI assignment)
LOGIC
This means patients with Connect will receive the questions 2 days prior to a scheduled visit to complete, and the questions will also be available in Welcome (iPad) or Hyperspace (care team to enter them)
The safety inbasket and pop up alert logic will be linked to the HARK and will function the same way it functions with the CMMI IPV questions

In the Preview Pane, if you add Questionnaires, you can see all questionnaires completed prior to the visit, including NYC Social Factors and NYC Hark

Additional important information
CLINICAL SCREENING
Depression, Substance Use, and Suicide screening results will still be visible in the Preview Pain,
Screenings, Flowsheets and Synopsis
They will NOT be reflected in the Storyboard or Wheel, and “Not on file” may show for those
domains in the Storyboard even if the questionnaires are completed
For this reason, it is important to always check the Preview Pane, Screenings tab, and/or Synopsis
IPV SCREENING
The HARK is the default IPV tool in Epic
This is not the tool we want to use in the future, it is a stopgap measure
The NYP Enterprise has requested that Epic consider updating the tool
