  • Schedule: Mon-Fri 1:00-6:00 pm (Practices) or until the end of home games
  • My Scope is the evaluation, treatment, and rehabilitation of sports related injuries and concussions with RTP protocols.
  • Baseline concussion testing can be requested through email
  • If your child sees a physician for any reason please make sure to get a doctor’s note stating:
    • Reasons for visit
    • If and when they can return to sport

Bullying – Granite SD

  • "Bullying" means intentionally or knowingly committing an act, regardless of whether the person against whom the conduct is committed directed, consented to or acquiesced in the conduct, that endangers the physical or emotional health or safety of a student or school employee

Bullying – Granite SD

  1. any brutality of a physical nature such as whipping, beating, branding, calisthenics, bruising, electric shocking, placing of a harmful substance on the body, or exposure to the elements;
  2. forced or involuntary consumption of any food, liquor, drug, or other substance;
  3. forced or coerced actions or activities of a sexual nature or with sexual connotations;
  4. other physical activity that endangers the physical health and safety of a school employee or student;
  5. physically obstructing a student’s or school employee's freedom to move; or
  6. emotional intimidation or exposure to ridicule; and
  7. is done for the purpose of controlling the student or school employee in some way or placing the student or school employee in fear of:

(i) physical or emotional harm to the student or school employee; or

(ii) harm to property of the student or school employee

Hazing – Granite SD

"Hazing" means intentionally or knowingly committing an act of bullying, as defined in this policy, which is done for the purpose of:

  1. initiation or admission into, affiliation with, holding office in, or as a condition for, membership or acceptance in any school or school sponsored team, organization, program, or event…

The conduct described above constitutes hazing, regardless of whether the person against whom the conduct is committed directed, consented to, or acquiesced in, the conduct.

Harassment – Granite SD

“Harassment” refers to unwelcome conduct targeted at an individual, or group of individuals, that is derisive, demeaning, or disparaging in nature and is based in whole or in part on the individual’s age, race, color, sex, pregnancy, religion, national origin, marital status, disability, sexual orientation, or gender identity, or because an individual conforms or fails to conform with a real or perceived stereotype.

“Sexual harassment” refers to unwelcome sexual advances, requests for sexual favors, or other verbal or written communications or physical conduct of a sexual nature when:

“Discrimination” refers to conduct including words, gestures, and/or other actions that unfairly disadvantages individuals based in whole or in part on that individual’s race, color, sex, pregnancy, religion, national origin, age, marital status, disability, sexual orientation, or gender identity, or because an individual conforms or fails to conform with a real or perceived stereotype.

Drugs, Alcohol, Tobacco – UHSAA Policy 10.1

The UHSAA supports the U.S. Supreme Court rulings regarding education. First, to prepare students to be good citizens and second, to teach them to be self-reliant and self-sufficient…

The use of alcohol, tobacco products or other drugs in almost every instance deters the realization of these goals…

Limitation for participation of students in Utah high school activities regarding the use of alcohol, tobacco products and other drugs during a sports season, as defined in the Utah Code are:

  • 1. First offense:
    • Suspension from two consecutive competitions
    • Practice may be continued following a personal assessment of the student by a licensed substance abuse intervention or treatment program.
  • 2. Second offense:
    • A six-week suspension from games, meets, matches, competitions or performances.
    • Student participation in an assessment by a licensed substance abuse intervention or treatment program with prescribed follow-up is required.
  • 3. Third offense:
    • An eighteen-week suspension from all games, meets, matches, competitions, performances and practices.
    • Reinstatement…successful completion of a formal assessment, intervention and treatment program…local school and/or district requirements which deal with discipline, suspension, corrective measures, parent/guardian involvement, rehabilitation and so forth, must be met.

Drugs, Alcohol, Tobacco

  • A. Conditions of this policy include:
    • The use of electronic cigarettes or vapor products are prohibited.
  • B. Implementation of Policy
    • Violations carry over year to year and sport/activity to sport/activity in a participant’s career (there is no “fresh start” each year).
    • Violations must occur and be discovered during a sport/activity season.
    • Any penalties for violation follow the student to any school to which he/she transfers.
    • Any violation beyond the third offense carries the same penalty as the third offense

    • Four additional years of either English, math, science, social science, world language, comparative religion or philosophy.

Name, Image, Likeness - Considerations

Although a student may accept money for the use his or her name, image, and likeness, doing so in the following manner will be considered a violation of the Amateur Rule:

1. Wearing a school team uniform or any identifying school insignia while appearing in any advertisement, promotional activity or endorsement for any commercial product or service; or

2. Lending his/her name and team affiliation for purposes of commercial endorsement. Any appearances by students for nonprofit organizations must be approved by the Board of Trustees.

**This provision is not intended to restrict the right of any student to participate in a commercial endorsement provided there is no school team or school affiliation

    • Prohibited Products: Deals forbidden for certain categories, including alcohol, tobacco, weapons, and gambling.
  • Disclosures are required for deals over $600 upon entering NCAA
  • Any appearances by students for nonprofit organizations must be approved by the UHSAA Board of Trustees.
