What is a permit ?

Permit Operation & Application:
PTW is a formal documents specifying the work to be done, hazards, and the precautions to be taken. The permits provides a clear written record, signed by a responsible person, supervisor, that all foreseeable hazards have been considered and all the necessary actions have been taken. It must be in the possession of the person in charge of the work before work can commence.

Personal Supervision | Supervision by a Person such that the supervising Person is at all times during the course of the work or testing is in the presence of the personnel being supervised. (face to face ) |

Types Of Permits
Permit for Work : “It is a safety document which is used when precautions need to be taken to achieve Safety from the System for work on any apparatus. It is designed to provide protection for employees who are working in hazardous situations”.
Limited Work Permit: “It is a safety document which is used to specify the limits of work or access when this alone is sufficient to achieve safety from the system. No isolation is required in it”.
Sanction For Test: “It is a safety document which is used when testing needs to be done on a system with adequate precautions need to be taken to achieve Safety from the System. To the system/ apparatus on which SFT is needed to be given, others permit need to be surrendered and this is only safety document issued”.

Types Of Certificates
Isolated: “Disconnected from associated Plant and/or Apparatus by an Isolating Device(s) in the isolating position, or by adequate physical separation or sufficient gap”.
Hot Work: “Hot work is any work that involves burning, welding, cutting, brazing, soldering, grinding, using fire- or spark-producing tools, or other work that produces a source of ignition”.

Excavation: “Excavation is the process of moving earth, rock or other materials with tools, equipment or explosives. Excavation is used in construction to create building foundations, reservoirs and roads”.
Radiation: “Radiation is the emission or transmission of energy in the form of waves or particles through space or through a material medium. This includes: electromagnetic radiation, such as radio waves, microwaves, infrared, visible light, ultraviolet, x-rays, and gamma radiation (γ)”.

General Instructions
Lock Out and Tag Out
Six Steps
Step 1: Preparation for LOTO
The first step of the locking and tagging out is the preparation phase. The SAP & AP must investigate and obtain a complete understanding of all kinds of hazardous energy that needs to be controlled. It is important to identify the particular hazards and of course, intends for the controlling or monitoring that energy. CP can also request for certain isolations pertaining to the nature of the job.
Step 2: Shut Down the equipment
With the planning complete, the actual or exact process of powering down and shutting off machines starts.
Shutting off command is initiated by a Control Person. An AP can also do the same if the equipment to be isolated is running on local state and if SAP nominates him.
Step 3: Isolate the Equipment
The next step of the Lockout/Tagout procedure is to isolate the machine and or the equipment from any source of energy. It starts with isolation from Electrical source of the equipment and draining / earthing of the power source if deemed necessary as per SAP instructions/IC certificate. Afterwards, Hydraulic, pneumatic, mechanical etc. energy sources are isolated and purging / venting carried out as per isolation certificate.

Step 4: Lockout/Tagout
With the machine or equipment separated or isolated from its energy source, the next step or the level of LOTO system is to lock and tag out the machine or device. Proper filling of the tags with name, sign and contact number is required.
During this step, the approved or the authorized employee will attach the lockout and Tagout devices to every energy-isolating device like motor breakers, valves, emergency push buttons etc.
Step 5: Stored Energy Check – Lockout/Tagout
Even after the energy source has been separated or disconnected, in step 3 (three) of the lockout safety process, and the machine has locked & secured out.
In step 4, that doesn’t fully guarantee that there’s no hazardous energy still stored or saved within the machine or that it’s safe to perform maintenance.
At this time, it’s important to look for any hazardous energy that’s been “stored” within the machine, or any “residual” energy. During this phase, any potentially hazardous stored or residual energy must be relieved, disconnected, restrained, or made non-hazardous in some other way. For example, Again opening of purge/vent valves can address the cause effectively.

Step 6: Isolation Verification – Lockout/Tagout
This last step of the Lockout/Tagout safety is all about making sure. Yes, shut down or turned off the machines are done, isolated them from their root of the power, locked them out, and inspected for hazardous stored energy.
At this point, an authorized person verifies the machine or devices have been suitably isolated and de energized.
A competent person (CP) is also required to check that all desired isolations have been performed in true spirit before the commencement of work at site.

5. The keys applied Key Safe Locks (Key Safe box Keys) shall be handed over to each competent person of each work section with the work permit having Key Safe Key # on it and shall be kept till the issuance of the permit.
6. After the completion of the job Key Safe Keys submitted one by one to SAP by the competent person of each work section while clearing the subject permit.
7. SAP removes the Key Safe Locks and opens the Key Safe as soon as he receives Key Safe Keys with the clearance of the permits.
8. After removal of Key Safe Locks, the isolation keys can be released for de-isolation by removing the control lock at the end.
