The general conference will be held live and in-person in Cleveland, OH, from October 27 - 29th.

Our theme this year is Amplifying Outcomes With Data

We are looking for interactive, quality session proposals that tell the data partnership stories from your community. Please note: The conference will be in-person only; there is no hybrid or virtual option for this content.

The deadline for all proposals has been extended to Sunday, June 21st, 2026, at 11:59 pm PT. Use this form to submit your session proposal. You may submit as many proposals as you wish.

The Set List (General Topic Areas)

We are requesting proposals that fit into the following general topic areas:

  • Cross-System Interoperability (The Backstage Pass): Show us how systems connect, such as closed-loop referrals to mainstream benefits and services or Medicaid integration.

  • Network Building (Building the Band): Successful cross-system collaborative models and effective partnerships, including tribal housing programs.

  • Prioritizing Subpopulations (The Opening Act): Strategies and best practices focused on specific groups, such as youth census counts or strategies to serve aging adults.

  • Creative Funding Solutions (Show Me the Money): Innovative approaches to resource allocation, like braiding funds or capital campaigns.

We strongly encourage you to submit your idea even if it does not neatly fit the topic areas suggested above. Proposals will be evaluated based on their relevance to the conference theme, clarity of objectives, and potential impact.

Conference Logistics & Policy

Presenter Perks (Discounts and Scholarships)

Presenters chosen for the NHSDC Conference will receive discounted event registration.

  • Discount Eligibility: Presenters must register for the conference by the registration deadline. For panel presentations, discounts are limited to three (3) presenters.

  • Scholarships: We are excited to offer scholarships to a small number of attendees. Due to high demand, we cannot provide them to everyone who requests one. If any of your speakers require a scholarship to attend, they may not receive one, and alternative arrangements must be made.

Protecting the Fans (Data Disaggregation)

All presentations must protect anonymity. All data must be aggregated and presented in a way that no reasonably foreseeable method could be used to identify any individual.

Promotion (Social Media/Marketing)

If your session is selected, many responses may be included in social media, conference materials, and other marketing tools, including:

  • Session Title

  • Session Description

  • Speaker information (name, organization, job title, biography, photograph, and social media handles)

Sound Checks (Modification of Selected Proposals)

The conference committee reserves the right to require modifications to content. Modifications may include asking presenters to revise their presentations or combine separate but similar presentations into a single presentation.

No Sell-Out Policy (Vendor Neutrality)

NHSDC strives to promote best practices, educate, and share data-informed results. To best foster such an atmosphere, NHSDC adheres to our vendor-neutral policy. Accepted sessions are required to comply, and each speaker will be asked to sign a document acknowledging their understanding of the policy before the session is presented at the fall conference.

You can read the full policy and see the acknowledgment here. A copy of the form will be provided at the conference for your signature.
