Under the terms of this Release and sufficiency of which is hereby acknowledged, the Releasor hereby releases and forever discharges the Releasee of: 50 Men Like King  from  all Liability.

THEREFORE under the terms of this Agreement and sufficiency of which is hereby acknowledged, do hereby release and forever discharge the Releasee including their agents, employees, successors and assigns, and their respective heirs, personal representatives, affiliates, successors and assigns, and any and all persons, firms or corporations liable or who might be claimed to be liable, whether or not herein named, none of whom admit any liability to the undersigned, but all expressly denying liability, from any and all claims, demands, damages, actions, causes of action or suits of any kind or nature whatsoever, which now have or may hereafter have, arising out of or in any way relating to any and all injuries and damages of any and every kind, to both person and property, and also any and all injuries and damages that may develop in the future, as a result of or in any way relating to the Liability.

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or
video tape without payment or any other consideration. I understand that my image may be edited, copied,
exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my
likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the
use of my image or recording. I also understand that this material may be used in diverse educational
settings within an unrestricted geographic area.

Photographic, audio or video recordings may be used for the following purposes:
• conference presentations
• educational presentations or courses
• informational presentations
• on-line educational courses
• educational videos

By signing this release I understand this permission signifies that photographic or video recordings of me
may be electronically displayed via the Internet or in the public educational setting.
I will be consulted about the use of the photographs or video recording for any purpose other than those
listed above.

There is no time limit on the validity of this release nor is there any geographic limitation on where these
materials may be distributed.

This release applies to photographic, audio or video recordings collected as part of the sessions listed on this
document only.

Due to the Coronavirus (COVID-19), 50 MEN LIKE KING is taking the prescribed precautions and establishing appropriate guidelines when having in-person events or activities with our members / customers.  The purpose of this document is written acknowledgment of both 50 MEN LIKE KING and individual participant’s understanding of the extended health & safety protocols in place at our events / activities.

In consideration of my participation in the 50 MEN LIKE KING EVENT the undersigned acknowledges and affirms the items below.  I am fully and personally responsible for my own safety and actions while and during my participation.  50 MEN LIKE KING cannot be held liable for any exposure to the COVID-19 virus caused by misinformation provided by the participants.  With full knowledge of the risks involved, I release 50 MEN LIKE KING, it’s Board, officers, independent contractors, and employees from any and all liabilities that may be sustained by me while participating in the aforementioned event or activity that may lead to unintentional exposure to COVID-19.

50 MEN LIKE KING enhanced COVID measures will include:

• All locations holding in-person activities will be properly cleaned and disinfected using prescribed guidelines
• Availability of hand sanitizer and / or disinfectant wipes
• Providing of masks to participants who do not have one
• Seating / table set up clearly marked and adequately spaced for social distancing

As a Participant, I will:
• as well as all members of my household, do not currently have, nor have experienced COVID-19 symptoms in the last 14 days
• have not knowingly been exposed to anyone diagnosed with COVID 19 in the last 14 days
• have not knowingly traveled to / from any regions, states, or countries designated restricted by public authorities in the last 14 days
• will follow social distancing guidelines
• will refrain from social norms of handshaking, hugging, etc.
• wash hands frequently
• wear a mask or face covering in all common, public areas as required
• will take my temperature at home, prior to attending the event / activity and affirm it is under 100.4
