Complete this form by the 25th of each month to be considered for that month's Gazette Advisor of the Month. Answer each question to the best of your ability.

Here are ideas of what each question can include:
Description of Who He/She Is can include what the advisor teaches, his/her hobbies, and/or facts about her childhood.
Why He/She Is a Role Model to Students and Adults can include qualities that are admirable and/or examples of interactions with students/adults.
Activities He/She Has Participated In/Implemented can include any activities the advisor has played a role in creating or implementing in the school or community such as starting a program or hosting an event.
