SMU Feral Cat Group Adoption Form
This is an initial screening and online adoption form for SMU Feral Cat Program rescue cats or special needs cats. Filling out this form does not guarantee you will be permitted to adopt one of our cats. 

All cats adopted through our program receive their initial vaccines at a wellness vet visit and are spayed/neutered before adoption. 

We require a $50 adoption fee that goes directly to our program and helps cover the cost of spaying/neutering and vet visits.

- I agree to provide the cat with proper shelter, nutrition, & humane treatment.
- I agree to adopt this cat as an indoor cat.
- We do not allow declawing (the amputation of the ends of the toes)
- I understand that pet ownership is a long‐term commitment (for the lifetime of the cat), many live to 15‐20 years.
- I agree to keep all immunizations/care current and up to date.
- I agree not to abandon, sell or surrender the cat to an animal shelter dispose of the cat in any way, and to notify SMU Feral Cat Program immediately if at any time I am no longer willing or able to keep the cat.
