EID US Product Replacement Form
This form is for EmergencyID customers from the United States who already own a customized product that references a now-defunct datafile. If you have multiple products you would like to replace, please complete this form once for each product, please do not fill out multiple engraving sections. Our Canadian customer support team will contact you to confirm the details of your replacement product(s). Replacement emblems and medallions will come with the jumprings required to attach them.
If you are a Canadian customer intending to request a replacement product, please do not use this form. Instead, follow the instructions in the emailed headlined 'Important EmergencyID Datafile Notice' sent September 10th.
