CHS Request for DL/Permit VOE
Please fill out the following form if you are requesting a Verification of Enrollment (VOE) for the purpose of receiving a Texas Drivers License or Permit.  Per state law, a student's attendance must be 90% or above to receive a VOE.  If attendance is less than 90%, then approval is required from the student's Assistant Principal.

Allow 24 hours for completion.

The VOE can be emailed for your convenience or picked up in the office, please specify.  VOEs are only valid for 30 days from date of issue.
