Common Health Challenge: Community Health Worker (CHW) Initiative Survey 

This survey aims to identify and learn more about organizations working in the United States that are:

  • Actively integrating CHWs across institutions and sectors
  • Supporting the Coalition’s mission of building bridges between health care and public health 

The findings will help shape the Common Health Challenge Catalyst Award RFP process, guide outreach to potential applicants, and identify technical assistance needs. Survey responses will remain confidential, and participants will receive an anonymized summary of results. To see the full list of questions in this survey prior to completing them, click here.

Who Can Take the Survey 

This survey is open to all organizations, both Coalition and non-Coalition members, that are involved in integrating Community Health Workers (CHWs) across sectors or working to strengthen partnerships between health care and public health. We welcome responses from a wide range of stakeholders, including but not limited to health care (hospitals, clinics, insurance providers), community-based organizations, and public health entities.
