2017-18 LHS Music Dept. Tour Signup
Our revised Spring Tour will be a 4-day trip to Southern California, which will include visiting Medieval Times, Disneyland, Universal Studios, and a college tour. We will include as many meals as possible; this is dependent on the number of students traveling.

Specifics:
Dates: Monday, Feb. 19-Thursday, Feb. 22, 2018
Cost: $750 students/$300 chaperones

Payment plan:
$100 NON-REFUNDABLE Deposit: due on or before October 27
$250: due by November 29
$250: due by December 20
$150: due by January 31

Payments can be made in cash, by check, or via PayPal (send payments to lincolnhsmusicboosters@gmail.com).
Once the deposit is received, participants will receive a packet to be filled out, which will include the tour contract and medical information forms.

Cancellation:
After Nov. 29, 2017: $200 cancellation fee
After Dec. 20, 2017: 50% non-refundable
After Jan. 31, 2018: 100% non-refundable
The cancellation policy will be strictly adhered to. No exceptions.

To reserve your spot on the tour:
1) Fill out this form (if you did it before, no need to do it again).
2) Turn in the $100 non-refundable deposit by Friday October 27th.

* If you gave a deposit for the cruise, we will assume that you would like to apply it towards this trip. If you no longer wish to go on the tour, please let your teacher know before Friday October 27th so that your refund can be issued.
