Canadian Bar Association - A.E.D. Order Form
Please use this form to complete your Automated External Defibrillator order. All sections must be completed in full. You will receive a response from our office within 2 business days of order placement.

NOTE: A non-refundable deposit of $500.00 is required to secure your order. This deposit can be payable by cheque only. This deposit will be managed by Peak Safety Services Legal Counsel in trust, and will be released to Peak Safety Services upon delivery of your order. You will be invoiced for the remaining balance NET 30 upon shipment of your order.

Phase 1 of ordering will run from November 1, 2016 to January 31, 2017 with delivery taking place by February, 2017.
