BORANG PERMOHONAN PENEMPAHAN PENGGUNAAN PERALATAN (RESERVATION APPLICATION FORM) FARMASEUTIKS, FAKULTI FARMASI, UiTM PUNCAK ALAM.
Perhatian (Attention) :

1. Borang permohonan mesti diserahkan ke Fakulti Farmasi selewat-lewatnya 1 minggu sebelum tarikh penggunaan.
2. Borang permohonan yang tidak lengkap dan permohonan melalui telefon tidak akan diproses.
3. Permohonan akan dipertimbangkan tertakluk kepada kekosongan peralatan.


1. Reservation form must be submitted to the Faculty of Pharmacy at least one week in advance.
2. Uncompleted reservation form or reservation through telephone will not be entertained.
3. Application will be evaluated upon instruments availability.

Peraturan-peraturan :
1. Di premis, sila log masuk sebelum memasuki makmal. Buku log akan disediakan untuk penggunaan peralatan. Mohon isikan semua maklumat dengan jelas pada setiap hari sehingga tarikh akhir penggunaan peralatan.
2. Diwajibkan memakai peralatan pelindung diri (PPE) yang sesuai (cth: Pakaian makmal,sarung tangan, kasut bertutup).
3. Pengguna tidak dibenarkan mengendalikan peralatan sehingga selepas menjalani sesi taklimat yang akan dikelolakan oleh staf makmal daripada fakulti. Selain itu, pengguna mesti mengikuti dan memahami SOP peralatan yang bersesuaian dengan sampel yang digunakan. Pengguna akan bertanggungjawab sekiranya berlaku kerosakan akibat kecuaian pengguna.  
4. Hendaklah mengemaskan meja, peralatan, dan sampel selepas menggunakannya.
5. Botol “WASTE” hendaklah dilabel,diisi paras maksima 3/4 dari saiz botol dan diletakkan di tempat yang dikhaskan.
6. Sisa klinikal/bahan hendaklah dibuang dalam tong hazard yang disediakan.
7. Botol/tong hazard yang telah penuh WAJIB diletakkan di pusat sisa bahan/klinikal/bahan kimia.
8. Tidak membawa barang yang tidak berkaitan ke dalam makmal.
9. Pihak Fakulti tidak bertanggungjawab ke atas kehilangan barangan peribadi di dalam makmal.

Rules and regulations :
1. At the premise, please log in before entering the laboratory. A log book will also be provided for the
use of the instrument. Please fill in all the particulars clearly on each day until the end of use.
2. It is compulsory to wear appropriate personal protective equipment (PPE) (e.g. laboratory clothes,
gloves, cover shoes).
3. Users should not handle the equipment until after a briefing session that will be conducted by the
laboratory staff from the faculty. Beforehand the user must go through and understand the SOP on the
equipment especially the suitability of sample type. The user will be held responsible if there is any
damage due to negligence of the user.
4. The workstation, apparatus used and samples should be kept neat and clean during and after the
experiment.
5. The "WASTE" bottle must be labeled, filled to a maximum of 3/4 of the bottle size and placed in a designated place.
6. Clinical waste / waste material should be disposed of in the hazard bin provided.
7. A full hazard bin / bottle must be placed at the chemical/clinical waste center.
8. Do not bring unrelated items into the laboratory.
9. Faculty of Pharmacy is not responsible for the loss of personal items in the laboratory.
