Equipment Sign Out Agreement
Dear parents and students,

As part of the classroom equipment sign out process, I want to make you aware of what is expected of your student who is presently enrolled in one of our Television Broadcasting Technology courses at Lenape High School.  Below is the procedure by which equipment is to be signed out and returned to me.  It is important to understand that any and all equipment that is signed out from Lenape is the responsibility of the students.  If signed out equipment is lost or damaged, then it is the student’s responsibility to pay for the repair or for a replacement.  Video equipment will not be checked out unless this form is submitted and the parent/guardian agrees to the understanding of responsibility.

Below is the procedure for signing out and returning equipment to the studio:

- Student makes a request for equipment to be used. Students may email the teacher to reserve equipment in advance.

- The equipment is then signed out in class or after school by Mr. Ebbeson or Ms. Rakow via barcode scanner.

- All video equipment is to be used over night or over a weekend and returned the FOLLOWING MORNING. For example if a camera is signed out on a Monday it needs to be returned on the following school day, which would be Tuesday.  Equipment will only be signed out over school holidays as deemed necessary.

- All equipment is to be returned the very NEXT school day by the start of 1st period to me in ND-25.  If a student is absent, then the equipment needs to be returned the day the student returns to school.

- If a student is absent for a lengthy period of time due to illness or other reasons, the equipment will be returned to the main office where I will pick it up and save any files if necessary.

- The equipment will be scanned in only by Mr. Ebbeson or Ms. Rakow and it should be returned to the proper storage locations immediately.

- If equipment is not returned by the beginning of 1st period, the student will be given a warning.

- If equipment is left at home or otherwise not returned the following school day then the student will be given a warning.

- If a student does not properly return equipment after they have already received a warning, that student may lose the privilege of signing out equipment.

Equipment is only to be returned to Mr. Ebbeson or Ms. Rakow and under no circumstances will any video equipment be given to a substitute or any other teacher.  If both Mr. Ebbeson and Ms. Rakow are unavailable, then the student needs to hold onto the equipment until it is able to be returned properly.
