This form is to schedule your Narcan Training. Please fill out the form below in it's entirety, choosing your top 3 best availability dates and times. We will accommodate your first choice as best as we can. You will have the option of over the phone, via zoom or in person ( here at the hospital, at Family of Ellenville or at the Ellenville Library).
A zoom link will be emailed 24 hours before the training.
**PLEASE NOTE - Narcan kits can only be guaranteed to Hudson Valley residents. If you reside outside of the Hudson Valley, you are more than welcome to attend but no kit will be provided.
