Please use this form to request a reimbursement check from the McAlpine PTO for your $100 Room Parent Fund allowance. NOTE: These PTO funds may NOT be used for the purchase of gift cards.
Upon completion of this form, you must email the receipt(s) for which you are requesting to be reimbursed as PDF or image files to the McAlpine PTO Treasurer (
mcalpineptotreasurer@gmail.com).
Please include your LAST NAME and "ROOM PARENT FUND" in the file name of your receipt(s) for faster processing.
Please do not submit receipts with personal items included! Please keep track of your spending (total budget for each classroom is $100). Please turn in all reimbursement requests by June 9, 2023.
Every effort will be made to provide your reimbursement check within 2 weeks.
