Volunteer Confidentiality Pledge

In my capacity as a volunteer at Short Years Diaper Bank I may learn personal facts about staff, volunteers, clients, and member agencies. I may become aware of confidential information related to the operation of the organization. I understand that I must exercise due diligence and caution in any discussion related to Short Years Diaper Bank and its' business.

I also understand that confidentiality is not limited to my current affiliation with Short Years Diaper Bank and that it continues in perpetuity.

Media/Photographic Release

SHORT YEARS PARTNERSHIP is granted permission to use my name, statement(s) and my likeness in any and all of its publications or advertisements in print, television, online (including without limitation, its websites or pages on Facebook or other social media sites) and any other media, without compensation to me. The consent is irrevocable and is without payment. The materials will become the property of the SHORT YEARS PARTNERSHIP and will bot be returned. The right to inspect or approve anything in which my name statement(s) or likeness appears is waived along with any right to royalties or other compensation arising or related to their use.
