The following is a registration and informed consent for our PT and OT Wellness sessions. These 45 or 60 minute sessions are provided by our team physical and occupational therapists and assistants and are offered on a first-come, first-serve basis each week when we have openings in our schedule. You’ll pay privately for these sessions, as you no longer demonstrate the medical-necessity required for insurance-covered care.
