2022-2023 Madrigal Feast Ticket Reservation Form
Please read the following instructions before ordering tickets.
  1. Please fill out the following form CAREFULLY. 
  2. Tickets must be reserved by Friday, November 18th.
  3. Tickets are $40 per person.
  4. Payments MUST be received by Karen Caruthers in the Southside High School Freshman Center Office.
  5. Please seal cash discreetly in an envelope. Make checks payable to Southside High School Choirs.
  6. You may purchase tickets for both evenings on one order form. However, so that others may see the show, we ask each guest to only attend one performance - not both.
  7. Please attach a note to your payment that includes: Purchaser Name, Phone Number, Email Address, Number of Tickets Reserved, Total Enclosed Amount.
