Please indicate all slots below during which your student is available. We expect demand to be very high for this session and the more availability you indicate, the more like we will be able to accommodate your student. We will let you know your student's assignment before registration for Session 2 of Future Stars enrichments opens.
What was your student's BOY iReady score in ELA? (You should have received this information from your student's teacher; if you didn't, you can ask for it or look it up through the iReady app in Clever after logging in as your student.)
Please give any other reading testing data you have for your student (iReady sub-scores, DIBELS, RI, etc). If you don't have any other testing data, just leave this blank.
Please tell us anything else relevant about your child's reading ability that it would be useful for their reading teachers to know in advance (e.g., strengths, weaknesses, confidence level).
Participation in the program typically costs $15/hour.

However, if your student receives a scholarship from Rising Stars Aftercare or the Future Stars Enrichment Program, received free lunch at school last year, or receives TANF benefits, their participation in the program is free.

Additionally, if $15 would represent a significant burden to your family and your child is currently behind grade level in reading, they can participate in the program entirely free of charge.
