I, the undersigned, acknowledge and understand that video and/or audio recording, photography, and live streaming (the “Media”) may be taking place on the premises of On Set Barber Shop for commercial, promotional, and entertainment purposes, including but not limited to television, social media, online platforms, and advertising.

By signing below, I hereby grant On Set Barber Shop LLC, its affiliates, licensees, successors, and assigns the absolute and irrevocable right and permission to record, use, reproduce, broadcast, and distribute my image, voice, name, likeness, and any statements made by me (collectively, the “Appearance”) in connection with the Production, in whole or in part, in any and all media now known or hereafter developed, throughout the world, in perpetuity.

I acknowledge and agree that:

I am voluntarily entering the premises and understand that filming is taking place.

I am not entitled to any compensation or credit for the use of my Appearance.

I waive any right to inspect or approve the final product or any use of my Appearance.

I release and discharge On Set Barber Shop LLC and all associated parties from any and all claims, demands, or causes of action in connection with the use of my Appearance, including without limitation claims for defamation, invasion of privacy, or right of publicity.
