This form describes the requirements for the course and asks you to commit to complete the requirements. If you are unable to make this commitment, we suggest you consider participating in the training program at a later time.
In order for participants of the DSP II Training to be considered credentialed as a DSP II, the following criteria must be met for completion:

- You should expect to spend between 1-5 hours per week outside of class completing the required work.
- There are 25 online lessons to complete through the Elsevier learning management system.
- There are 4 out-of-class assignments to complete.
Requirements for receiving the DSP II credential:
- All assigned online lessons must be completed by their due dates. There are 25 lessons to complete.
- All online post-tests must be passed with a score of 80% or higher.
- All in-person class sessions must be attended with the exception of 1 excused absence for monthly class participants (2 excused absences for twice a month class participants)
- All absences must be communicated at least 1 week in advance of the absence; or in the case of an emergency, within 24 hours of the class session that will be missed.
- In the case of any absence, all assignments must be complete and submitted in accordance with deadlines stated on the training guides and it is the responsibility of the participant to coordinate with a peer and/or trainer of the training program to receive any class notes, handouts, etc.
- All mandatory assignments listed on the training guides and/or assigned during the in-person class sessions must be complete and submitted by their due dates.
- The final exam must be passed with a score of 80% or higher.
Should any of the above criteria not be met, the participant will be ineligible for DSP II credentialing and the DSP II Mentor track.
