AWANA Registration Form First Baptist Moreland (2930 Hwy29, Moreland, GA)
AWANA registration at First Baptist Church Moreland is OPEN!  Please register your child below!  We look forward to seeing you at the first AWANA meeting on August 19th from 6pm-7:30pm! Optional Wednesday night dinner (with suggested donation of $5 each or $20 max per family) from 5:20pm to 6pm. 
For child drop off, walk your child inside and sign them every Wednesday evening. For pick up, walk in and join us inside the Fellowship Hall to pick up your child at 7:20pm. If transportation for your child changes, it is important to let us know who is picking up your child.  
Cost for Awana:
-Every child registered needs to purchase a book.
-The books are $10 each

UNIFORMS REQUIRED FOR AWANA ARE $15: (If your child is new to Awana or moving up to a new age group, they need a new uniform purchased by the parent.)
During the year, your child will work hard to earn badges and awards to display on their uniforms and vest. Uniforms are required to be worn to Awana.
Ages 3 and 4 (Cubbies): Uniforms are $15
Grades K to 2nd grade (Sparks): Uniforms are $15
 3rd to 5th grade (T&T): Uniforms are $15
Youth (grades 6th to 12th) optional t-shirt is $15 (Youth are not require to wear a uniform)
**No one family pays more than $60.00 for uniforms and books

I give my permission for my above named child to participate in First Baptist Church AWANA activities and other activities held by or at First Baptist Church in Moreland (FBCM).  I hear-by release, the AWANA staff and First Baptist Church Moreland from liability for any injury that my child may sustain during club meeting and/or additional AWANA sponsored or First Baptist Church or Moreland sponsored activities. I authorize the staff of AWANA Clubs to obtain whatever emergency medical attention might be necessary for the above named child in the event I cannot be reached.
