This approval step also brings us to our final payment milestone in the project timeline, if applicable.

By entering my name and submitting this form, I affirm that I have reviewed the above design proof(s) thoroughly and authorize Unity Design to proceed with the next phase in the process. I understand that these are "soft proofs," and are a close but not exact representation of the final print. Because of the differences in equipment, screen capabilities and calibration, paper types, inks, printer capabilities and settings, and other conditions and variables that exist between what you see on your screen and press room operations, a reasonable variation in color between proofs and the completed job are to be expected and shall constitute acceptable delivery. I acknowledge that I am responsible for insuring that all content contained in these proofs is correct and complies with all federal, state, and local government regulations. I acknowledge that I am responsible for obtaining rights to use any text, photos or original art submitted for publication, and that Unity Design is not responsible for obtaining permission to use intellectual property. Although Unity Design checks carefully through all stages of production, I accept full responsibility for any errors or omissions in the approved proof(s). I further acknowledge that Unity Design cannot be held liable for any misfortune that may arise from the use of the approved designs beyond the project fee amount. Design modifications requested after this approval form is submitted, and any additional prepress work required as a result, will delay our completion date and are billed at the standard hourly rate of $150 in 15-minute increments. Please keep your press-ready files archived for future use. Requests for press-ready files after the first set are delivered can be accommodated for $25 per file. Any fees incurred beyond the initial project price must be paid in full before press-ready files are released to the client.
