In case of a serious accident or illness, the services of a hospital, physician, dentist, or any other related service (such as EMS) will be acquired and the charges will be made to the parent/guardian.  Reasonable attempts will be made to contact parents/guardians prior to services, whenever possible.

CONDITIONS OF ENROLLMENT IN CAMP
 
The Choir Director, or her designate, reserves the right to dismiss a chorister who, in her opinion, is a hazard to the safety and rights of others, or who appears to have rejected the reasonable controls of the Camp.  Sometimes campers must be dismissed because they have behavioral, physical, emotional or social difficulties of a degree that the camp staff is unable to manage appropriately in our camp context.
Every precaution is taken for the safety and good health of the Choristers, but in the event of an accident or illness, Camp Horizon, Calgary Children’s Choir Board of Directors, Chaperones and Staff are hereby released from any liability.
In case of an emergency, I understand that every effort will be made to contact the parent(s) or guardian(s).  In the event I cannot be reached, I hereby give permission to the physician selected by the Choir Director, or her designate to secure proper treatment for my child.
