Students requiring instruments may use school owned instruments, provided that they are available. Any student using a school owned instrument must first fill out and submit a Rochester Community Schools Instrument Use Contract. Any student using a school owned instrument should understand that repairs due to everyday wear and tear will be covered and paid for by the school. Any damage to (including dings, dents, or scratches) or loss of any instrument due to gross negligence or carelessness will be the financial responsibility of that student and their family.

Submission of this Form constitutes the acceptance of the responsibility for the daily care and maintenance of the instrument described in this contract. This instrument must be maintained and returned in the same condition in which it was issued. It is understood that the school does not carry insurance on this instrument and therefore any loss or damage is solely the financial responsibility of the undersigned. Failure to do so will result in the undersigned being charged for repairs if possible, or replacement if necessary.
