Activities Disclaimer/Agreement (under 18 years - parent/guardian consent required): You understand by enrolling your child, that there are inherent risks involved with participating in our activities, including but not limited to minor injuries, serious injuries, death and or otherwise that could/may occur as a result of your child attending and whilst being involved and participating in any of our activities, projects, events or otherwise. You agree that you know of No known reasons why your child is not able to carry out any of our activities, projects, events and or otherwise and confirm they are medically fit and well to carry out physically demanding activities. If you have any concerns, you should contact the child’s medical practitioner immediately before commencing any of our activities, projects, events and or otherwise. By signing below, you agree to your child participating in any of our activities and understand the risks involved and agree to waiver, discharge and release Little Enablers (Enable You Services) and or it’s staff and third parties involved from any claims, defamation and or liabilities of any kind and actions. By any minor injuries, serious injuries, death and or otherwise as a result of carrying out our activities that may occur towards your child, whilst participating in Little Enablers (Enable You Services) activities, projects, events and or otherwise. By signing and submitting this form you agree you fully understand the risks involved and that which is towards your child and understand this disclaimer/waiver of liabilities. You confirm by signing that you have the authority and have full capacity to sign this registration form. By signing below, you agree you fully understand this disclaimer/waiver declaration and give consent for this disclaimer/waiver to stand from signed today’s date and be effective for all other future activities, projects, events and or otherwise that your child may attend and or participate in with Little Enablers (Enable You Services) in the future. By signing below, you consent for emergency first aid to be administered to your child in the event of a medical emergency and agreeing where required for the appropriate emergency services to be summoned. By signing below you agree to pay the required service fees for your child to attend our sessions, before the start of each session, payable monthly in advance. By signing below you agree to adhere to Little Enablers (Enable You Services) policies, guidelines and procedures (including but not limited to our behaviour/attendance policy) at all times (which can be made available upon request). Behaviour management: We want to ensure everyone attending our sessions are in a safe and caring environment at all times, to help with managing this if a child receives a number of warnings regarding unacceptable behaviour (including but not limited to hitting, biting, kicking, swearing, bullying) during any session, which is deemed unacceptable, we will notify the parent/guardian of this behaviour and ask for the child to be collected and or ask the child to sit out the session for part or all as required. In serious or reoccurring cases of unacceptable behaviour, LittleEnablers (Enable You Services) staff reserve the right at anytime to stop participation and or withdraw your child’s place with immediate effect with no refund of session fees. This is to help maintain safety of all children/staff at all times. Any verbal, physical and or abusive behaviour from any adults will not be tolerated at anytime and the appropriate authorities, maybe summoned as required and may result in your child’s place being withdrawn without refund of any session fees. Attendance guidelines: If your child misses two sessions without reasonable cause, deemed appropriate by LittleEnablers (Enable You Services) staff, your child will be removed from our register and no longer have a place, without notification and you will have to completed the registration process again, this is to ensure fairness and places are available for children that wish to attend. We do not normally operate sessions over the school half terms, unless in exceptional circumstances. *Please note: WhatsApp is the preferred method of communication, we may from time to time offer communication using different methods, we do not take responsibility or accept any and or losses or liabilities of any kind as a result of missed, misused, lost, damaged, corrupted or unreceived information from us to you and device equipment you use, you understand you do so at your own risk at all times. Cancellations: In the event a session can not go ahead due to unforeseen circumstances, we will try to notify you as soon as possible (normally via parent WhatsApp Group) however this may not always be possible, in the event of a cancelled session, no fee will be due for that session only and may be rolled over to the next session, LittleEnablers (Enable You Services) does not accept any liabilities as a result of losses at anytime including occurred by attending cancelled sessions. If your child no longer requires their place, please give at least one week’s notice, failure to do so may result in payment being required for sessions not attended. This disclaimer/agreement is governed by and shall be construed in accordance with the laws of the United Kingdom.
