Facility Rental Request Form
If you'd like to submit a request for one of our facility spaces to rent for a baby shower, birthday party or other event, please submit this form. All requests are tentative until you receive confirmation via email from the parish secretary.

Have you signed a Facility Use Agreement with the parish secretary? Your rental will not be confirmed until this document is signed. *
The fee to rent McNally Hall is $100. This covers the cleaning cost, and is due at least one business day prior to the event. *
Renters must provide a certificate of insurance for $1 million for the rental event with St. Barnabas Catholic Church listed in writing on the certificate as the insured party.
*The liability insurance recommended by the Archdiocese can be found online at https://www.kandkinsurance.com/sites/Tulip/Pages/DioceseEligibility.aspx
Liability insurance certificate must be submitted prior to the event, or the event risks cancellation.
