AWHA Dashboard - Monthly Reporting Form
Please report each month's data by the 15th of the following month. (ie. January's data should be reported by February 15th.)

If your community is not collecting a particular data point, you may just leave the field blank.

For questions about completing this form, please contact Megan Gibbard at A Way Home America (megan@awayhomeamerica.org) and Margaret Woley at MEMconsultants (margaret@memconsultants.com).

For the purposes of this project, "youth" refers to unaccompanied youth and young adults between the ages of 12 and 24. If your community has a different definition for youth, please let Megan and Margaret know.
