Anthem Client Reporting Request Form
Please complete the following fields for each client requesting reporting.

Instructions and Delivery Timing
* Submit one form per client
* After a form is submitted, monthly and quarterly client reports will be generated on an ongoing basis (no need to resubmit)
* Client groups will need a minimum of one full month of 30 or more members enrolled in Lark to receive a monthly report, regardless of when the reporting is requested (i.e., a reporting request form submitted on May 7th may not be delivered until July 15th, depending on enrollment)
- Monthly reports are delivered on the 15th business day for the prior month, i.e., January report delivered February 15th
- Aggregate reports for all groups will be provided if there are 30 or more members enrolled
- Individual reports per group will be provided for each group with 30 or more members enrolled
* Client groups will need a full calendar quarter of 30 or more members enrolled in Lark to receive a quarterly report, regardless of when the reporting is requested (i.e., a reporting request submitted in March would be delivered in July for Q2)
- Quarterly reviews are delivered by calendar quarter on the 15th business day of the month following the quarter-end:
- April 15th for Q1
- July 15th for Q2
- October 15th for Q3
- January 15th for Q4
