This form is for registering students who already applied and were accepted.
This form must be submitted for each child who is being registered.
After submitting this form, please bring the following to the office in order to complete the registration:
Immunization Records Discipline Record Attendance Record Proof of Address Birth Certificate (or Passport) Valid, government-issued picture ID of the parent/legal guardian Most recent transcript, report card, progress report or grade report Copies of IEP, 504, or ALP plans, if applicable Home Language Survey (1) Non-refundable student fee of $50.00 per year (for grades 1st – 12th) (2) First installment of the Kindergarten fee of $300 per month (for Kindergarten only) (3) Chromebook insurance fee of $25.00 per year (for Grades 9 - 12) Internet Acceptable Use Agreement (4) New Student Statement (5) Authorization for Release of Records (6)
(1) This survey is required for students who have NEVER been enrolled in an APS school at any time during their school years. A copy of this survey can be obtained from the registrar.
