Highland Food Service Request Form
The Lewis County C-1 School District is dedicated to trying to provide Highland children with meals during this extended school closure related to COVID-19. Due to LCC1's involvement in the Summer Food Service Program during the summers, we believe we will be able to provide any child under the age of 18 in our school district boundaries with MEALS AT NO CHARGE to the families during this school closure. Our plan is to provide Breakfast & Lunch to those that signup on a daily basis during the current school closure.

This survey is to gather the necessary information to plan and arrange for the delivery of meals to children with a hopeful begin date of Monday, March 23, 2020. Please understand that this start date is tentative, but we are hoping to make meals available starting on this date.

This survey only needs to be completed once to sign your children up for meal delivery service!

Meal delivery times will be determined by the number and location of the children enrolled in the program.
Participants will be contacted at a later time to communicate the expected delivery time.

PLEASE RESPOND BY 10:00 AM ON FRIDAY MARCH 20TH to be enrolled in time for deliveries expected to be made on Monday March 23rd!
