Young This agreement between me, my parents or guardian and the administration, sponsors and coaches at Capital High School is designed as a multi-purpose instrument to reinforce the efforts made by individuals to appropriately and positively represent themselves and Capital High School in all phases of performance in all endeavors sponsored by Capital High School.
Capital High School has, since its establishment, been recognized for excellence in the arts, athletics and academics. It is that very recognition that brings this agreement to life. The privilege of wearing a Capital High School uniform or performing under the name of Capital High School requires that the student continually represent themselves, their team mates, their parents, their coaches/sponsor and Capital High School in a positive manner. Students who participate in these activities must demonstrate positive behaviors at all times, not simply during participation in their respective activities.
Any student, who wishes to participate in any organization, group, team, event, game, contest, competition and the like must willingly accept the responsibility that accompanies the privilege of representing Capital High School. The following stipulations are to be adhered to by all students:1) All students participating in both co-curricular and extra-curricular activities will maintain a 2.0 grade point average and must maintain a passing grade in all classes to remain eligible to participate in any type of activity that would require participation after school or at any other time. 2) It will be the duty of the coach or sponsor, teachers, administration and parents to assist the student achieve a passing grade in all classes. Any student who may be failing a class will enter a probationary period (approximately two weeks) in which they must demonstrate academic improvement. If improvements are not made the student will be ineligible to participate in practices, games or performances until such improvement (a passing grade) is attained. 3) All state, county, and school laws, policies, procedures, and rules will be followed. Violations will warrant consequences that will be applied accordingly. 4) All students who participate in an event on an evening prior to a school day will attend school the following day on time, all day and with all assignments complete.5) All participating students will conduct themselves in a manner that positively reflects upon themselves, their team mates, their parents, their coaches/sponsors, and Capital High School. Major Disciplinary Infractions will not be tolerated! (See the Student Handbook). Major Disciplinary Infractions for the purposes of this agreement shall also include tardiness to class and/or school, skipping class and/or school. Severe consequences should be anticipated and may include suspension from practices, performances, and games or dismissal from the team or organization altogether. Commission of a level three and/or a level four offense may be grounds for immediate dismissal from a team or organization. 6) Physical or verbal confrontations and fighting or the provocation thereof on school property or any public place will result in consequences which may include suspension/expulsion from school and dismissal from a team or organization. 7) It is expected that the parent(s) will accept the duty, obligation, and responsibility to conduct themselves in a manner befitting a responsible adult. Parents will not question the authority or decision making of a coach, sponsor or teacher in a forum that would tend to undermine the authority of that coach/ sponsor or teacher.

I also agree to all conditions, terms and stipulations set forth in this Assurance Agreement.
