Dr Kaliye, Dr Afzal and Dr Wakaas are currently accepting new patients. You may view their bios on the Doctors section of our website to help you choose who you would like to register with.

New patients are encouraged to register by completing the meet & greet appointment, once registered you  can see any of our physicians as needed, helping ensure timely access to care.

Patients who prefer a female physician are encouraged to register now  and complete a meet and greet any of our doctors to ensure access to the Family Practice, with the option to transition once her roster becomes available.

If you would like to be notified when her roster opens, please indicate this at the end of the form or mention it during your meet and greet appointment.
