2018 Flag City Balloon Fest Flight Crew Volunteer Registration
Thank you for volunteering for the Flag City Balloon Fest. Our success is tied directly to the incredible
community support we have every year.
The form below is required for participation on a balloon crew and helps us assign you to a balloon and
pilot.
A separate form is required for each person. All volunteers under the age of 18 must have an adult on the chase team with them. (a 1:1 ratio is required).
Note that your pilot may require an additional waiver to be signed for him or her at the time of flight or
inflation.

RELEASE AGREEMENT
FLAG CITY BALLOON FEST WAIVER, RELEASE AND HOLD HARMLESS AGREEMENT
In consideration and allowance for being granted permission to engage in activities associated with the Flag
City Balloon Fest and/or other activities of Flag City Balloon Fest Inc. (herein referred to collectively as "Flag
City Balloon Fest" and together with myself, "The Parties"), I, the undersigned, hereby agree for myself and
for my personal representatives, heirs and next of kin:
By signing this Release Form, I release Flag City Balloon Fest, its officers and directors, staff, agent,
employees, volunteers, hosts of the event, sponsors, and its successors or assignees from and against any
and all liability, including liability for negligence, and acknowledge Flag City Balloon Fest is disclaiming all
warranties.
NOTICE OF RISK: I understand that participation in activities may be dangerous and may expose me to risk
of serious bodily injury and possibly death. These risks include, but are not limited to, the possibility of
collisions with other participants, spectators, and the public, property while participating in such activities. I
understand that no degree of care or caution can completely eliminate these risks.
ASSUMPTIONS OF RISK, RELEASE OF LIABILITY, REQUIREMENT TO MAINTAIN INSURANCE: I
hereby freely and expressly assume and accept any and all risk of injury and/or death arising from my
participation in any and all activities I may undertake in conjunction with the Flag City Balloon Fest by or
through Flag City Balloon Fest. I hereby release Flag City Balloon Fest from liability for any and all injuries
and damages, including death, arising from my participation in any and all activities I may undertake. In so
doing, I release Flag City Balloon Fest from all liability, including, but not limited to, liability for negligence,
failure to warn, product liability and warranty arising out of or in any way connected with my participation in
activities. Furthermore, I promise and agree not to make any claim or commence any lawsuit against Flag
City Balloon Fest for injuries or damages arising from my participation in such activities.
PHOTOGRAPHIC RELEASE: I hereby grant Flag City Balloon Fest permission to use my likeness in a
photograph in any and all of its publications, including website entries, without payment or any other
consideration. I hereby irrevocably authorize the Flag City Balloon Fest to edit, alter, copy, exhibit, publish
or distribute this photo for purposes of publicizing the Flag City Balloon Fest or for any other lawful purpose.
In addition, I waive the right to inspect or approve the finished product, including written or electronic copy,
wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph.
INDEMNITY AGREEMENT: I hereby agree to indemnify Flag City Balloon Fest for death or injury to myself
or any other person or property arising from my participation in activity. I have carefully read this agreement
and fully understand its contents. I am aware that this is a release of important legal rights and a disclaimer
of important warranties, that I am signing this agreement in consideration of my participation in activities,
and that this agreement is an enforceable contract between myself and Flag City Balloon Fest, which I sign
freely and voluntarily. This Agreement contains the entire agreement between THE PARTIES to this
Agreement and shall not be modified in any manner. The laws of the State of Ohio shall govern the validity,
performance and enforcement of this agreement. Should either party institute legal suit or action, it is
agreed that the venue of such suit or action shall be in the County of Hancock, State of Ohio USA. In any
provision of this agreement is held in whole or in part to be unenforceable for any reason, that provision will
be severable and the remainder will remain enforceable and in effect.

FLAG CITY BALLOON FEST BALLOON CREW VOLUNTEER GUIDELINES
Thank you for volunteering for the Flag City Balloon Fest. Below are the GENERAL RULES for you
participation as a Balloon Crew for the Flag City Balloon Fest. These Rules must be followed while you are
acting as a participant in the Flag City Balloon Fest.
VOLUNTEER RESPONSIBILITIES:
Assist in the set-up and retrieval of the balloon you are assigned to. MUST follow the directions of the pilot
in command/crew chief of his balloon. Requires some heavy lifting.

GENERAL RULES
The Pilot is in command.
Listen to the Pilot's instructions. If you do not understand, ask again.
Never agree to crew if you have been drinking in the last 12 hours.
Never smoke around the balloon.
Wear appropriate clothing and closed footwear. You may have to walk through patches of weeds and wet
ground. Shorts or sandals are not appropriate. Dress in layers for those early morning flights.
Wear gloves to protect yourself and the envelope(Balloon). Don't pack the gloves up with the equipment
after the launch. You will need them for landing.
Never promise a ride to someone.
Locate the fire extinguisher and know how to use it.
Be careful around domestic animals at the landing area.

BEFORE THE LAUNCH
Arrive on Time. Late arrival could cause the flight to be cancelled.
Check the launch field for anything that could damage the envelope.
Unload the equipment carefully. Do not lift more that you can handle.
Remind spectators to stay clear of the equipment and away from the tie-off lines, crown lines, etc.
Remind them of the NO SMOKING Rule.
Do not wrap lines around your hands, arms, or body.
Don't allow yourself to be lifted by any of the lines or by the basket once the launch has started.
Keep your feet out from under the basket.
Note the time of launch.

AFTER THE LAUNCH
Load the equipment carefully.
Inspect the launch site for equipment (gloves, lines, cameras, phones, trash, etc.)
Inspect the launch site for litter.
Leave the launch field carefully so as not to leave ruts or damage to the field.
THANK the landowner if in attendance.
