This is the registration form for OIS Training with Among Friends, LLC.  Please complete the form and make your payment to secure your spot in the training.

Organization *
Enter the participant's organization, agency, or company name.
First Name *
Enter the participant's legal first name.  This will be the name shown on the certificate.
Last Name *
Enter the participant's legal last name. This will be the name shown on the certificate.
Nick Name
Enter the participant's nick name, if applicable.  This will be the name used in class.
Organization Phone *
Enter the organization's area code and phone number (XXX-YYY-ZZZZ).  This will only be used if needed regarding payment.
Participant Phone *
Enter the participant's direct (EMERGENCY) phone number with area code (XXX-YYY-ZZZZ). This will be used in case of emergency or cancellation.  If you do not provide an emergency phone number, then Among Friends and Shanya Luther are not responsible for notifying the participant.
Invoice Email Address *
We will send the invoice to this email.
Certificate Email Address *
If the participant passes the class we will send the final certificate to this email. (or an explanation of why we could not pass this participant)
Participant Email Address *
If the participant passes the class we will  also send the final certificate to this email. (or an explanation of why we could not pass this participant)
OIS *
Has the participant taken an OIS Training in the past?
Attire *
By checking the button below, you verify the participant understands they must wear comfortable, athletic style clothing and closed toed athletic shoes (ie: "tennis" or running shoes)
Certification *
By checking the button below, you verify the participant understands that IN ORDER TO BECOME CERTIFIED they must: arrive on time and be present both days of the training, wear the appropriate attire, participate in activities and discussions, demonstrate they understand and can apply the concepts of OIS and be able to perform the physical skills portion of the training.   Certification is at the discretion of the trainer and is not guaranteed simply by attendance.
Payment *
By checking the button below, you verify the participant understands that payment for the training must be received in advance.  If they have not paid in advance, they will not be able to attend the training unless they have negotiated an alternative payment plan with our registrar.  The cost for the class is $105 per person and an invoice will be sent to the email you provided.
Refunds *
By checking the button below, you verify the participant understands that the fee will not be refunded.
