Dear Parent/Guardian,

In preparation for the 2024-2025 school year, please complete the form below with your child's transportation needs for the upcoming school year. If you have more than one student in your household, please complete a form for each student. Remember that students may only have ONE morning stop and ONE afternoon stop. We do not accept "As Needed" Transportation Requests.

It is imperative this information be completed as soon as possible in order to assist us in completing accurate and efficient bus routes over the summer. Students for whom we do not receive information will be routed as they are in our system now (which could be inaccurate).
