Declaro, por meio deste termo, que concordei em participar da presente pesquisa desenvolvida pelo grupo de pesquisa intitulado Rede de Pesquisa em Conhecimento e Tecnologias da Universidade Federal da Bahia (REDPECT-UFBA) . Afirmo que aceitei participar por minha própria vontade, sem receber qualquer incentivo financeiro ou ter qualquer ônus e com a finalidade exclusiva de colaborar para o sucesso da pesquisa. Fui também esclarecido(a) de que os usos das informações por mim oferecidas estão submetidos às normas éticas destinadas à pesquisa envolvendo seres humanos, da Comissão Nacional de Ética em Pesquisa (CONEP) do Conselho Nacional de Saúde, do Ministério da Saúde. Minha colaboração se fará por meio de questionário.  O acesso e a análise dos dados coletados se farão apenas pelos pesquisadores membros do grupo de pesquisa REDPECT - UFBA. Fui ainda informado(a) de que posso me retirar desse questionário a qualquer momento, sem prejuízo para meu acompanhamento ou sofrer quaisquer sanções ou constrangimentos.

I declare, through this term, that I agreed to participate in this research carried out by the research group entitled Research Network in Knowledge and Technologies of the Federal University of Bahia (REDPECT-UFBA). I affirm that I agreed to participate of my own free will, without receiving any financial incentive or having any burden and with the sole purpose of collaborating for the success of the research. I was also clarified that the uses of the information offered by me are subject to the ethical standards for research involving human beings, of the National Research Ethics Committee (CONEP) of the National Health Council, of the Ministry of Health of the Federative Republic from Brazil. My collaboration will be done through a questionnaire. Access and analysis of the collected data will only be done by researchers who are members of the REDPECT - UFBA research group. I was also informed that I can withdraw from this questionnaire at any time, without prejudice to my monitoring or suffering any sanctions or constraints.
