Jasmine Journey Holistic Wellness Center, LLC

1. Release of Liability & Indemnification: I hereby release, waive, and discharge Jasmine Journey Holistic Wellness Center, LLC, its owners, officers, and employees, specifically including Natalie Luna, Sahzira Schoby, and Zaiden Schoby (collectively, "Released Parties"), from any and all liability, claims, or causes of action arising out of any property damage, personal injury, or death that may occur while my child is participating in camp activities, whether caused by the negligence of the Released Parties or otherwise. I agree to indemnify and hold harmless the Released Parties from any loss or costs they may incur due to my child's participation.

2. Assumption of Risk: I understand that camp activities involve both indoor and outdoor environments, including a private grassy backyard. I acknowledge that these activities carry inherent risks, including but not limited to: heat-related illness, insect bites, allergies to grass/plants, trips, falls, and physical exertion. I voluntarily assume all risks associated with my child’s participation.

3. Medical Consent & First Aid: I authorize Natalie Luna, Sahzira Schoby, or Zaiden Schoby to administer basic first aid for minor injuries, including cleaning and bandaging minor cuts, scrapes, or bruises. In the event of a medical emergency, I authorize the staff to seek professional medical treatment and/or transport my child to the nearest medical facility. I understand that I am solely responsible for any costs incurred for such medical treatment or transportation.

4. Topical Application & Personal Care: Parents/Guardians are required to provide their child with their own labeled bottle of sunscreen and bug spray daily. I authorize camp staff to assist my child in the re-application of their provided products. In the event that my child’s provided products are forgotten or depleted, I grant permission for camp staff to apply "on-hand" products (standard SPF 30+ and DEET-free repellent). I have listed any known skin sensitivities or brand-specific allergies in the "Allergies" section of this form.

5. Timely Pick-Up & Late Fees: Camp concludes strictly at 3:00 PM. In the event of an unavoidable delay, parents are required to call or text 281-731-3007 immediately. I understand that a late pick-up fee of $1.00 per minute will be assessed starting at 3:10 PM. This fee is charged at pick up using the card on file, unless you indicate a desire to use an alternate payment method and the fee is non refundable. 

6. Operational Policies: I understand that camp is a full-day experience. Early departure is permitted; however, I must notify Natalie Luna at morning check-in. Registration fees are fixed and do not change for early departures.

7. Registration & Financial Policy: I acknowledge that a spot is NOT reserved until the full registration fee ($250 per child / $225 per sibling) is received via Venmo (@JJHOLISTICWELLNESS) or a paid electronic invoice. Once registered, a credit card will be requested to hold on file in the event of late pick up charges accrued. 

8. Refund & Transfer Policy: I acknowledge that all payments are non-refundable. In the event my child cannot attend, the registration is transferable to another child or a future 2026 session, subject to availability and prior approval from Jasmine Journey management.

9. Legal Signature: By checking the "I Agree" box below, I confirm I have read this agreement in its entirety, understand its terms, and am providing my legal consent for my child to participate.
