In consideration of being allowed to participate in (“Activity”), to the extent permitted by law, I hereby forever discharge, release, and hold harmless School District No. 1, in the City and County of Denver and State of Colorado (the “District”), and its employees, officers, directors, agents, representatives, and authorized volunteers (collectively, the “Releasees”) from any and all liability, claims, demands, actions, and causes of action arising from or related to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in the Activity.

I am fully aware of the potential risks and hazards and I hereby elect to voluntarily participate in the Activity. I knowingly and freely assume all potential risks, known and unknown, including those arising from the negligence of the Releasees.

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, further agree to indemnify and hold harmless the Releasees from any loss, liability, damage or costs, including court costs and attorneys’ fees, that Releasees may incur due to my participating in the Activity, including those caused by the negligence of the Releasees or otherwise.

I have read this Waiver and Release of Liability (“Agreement”) and fully understand its terms. I also understand that I have given up substantial rights by signing it and sign it freely and voluntarily without any inducement. I am at least eighteen (18) years of age and fully competent to sign this Agreement.

(* If Participant under 18 years of age.  It will also be presumed that the signing Parent/Guardian has legal authority to sign on behalf of minor participant.  If another individual has legal decision-making authority, either all Parents/Guardians must sign or the signing Guardian is legally attesting under penalty of perjury that they have joint decision-making authority and have received permission from the non-signing Parent/Guardian to sign on his/her/their behalf.)

A fin de que se me permita participar en ______________(la “actividad”), en la medida permitida por la ley, por este medio libero, exonero y amparo para siempre al Distrito escolar n.º 1, en la ciudad y el condado de Denver, Estado de Colorado (el “Distrito”) y a sus empleados, funcionarios, directores, agentes, representantes y voluntarios autorizados (conjuntamente, los “exonerados”) ante cualquier y toda responsabilidad, reclamos, demandas, acciones y causas de acción que se deriven o relacionen con cualquier daño, pérdida o lesión, incluyendo la muerte, que pueda sufrir mi persona o cualquier bien que me pertenezca mientras participe en la actividad.

Soy plenamente consciente de los posibles riesgos y peligros y, por este medio, decido participar de manera voluntaria en la actividad. Consciente y libremente asumo todos los posibles riesgos, conocidos y desconocidos, incluyendo aquellos derivados de la negligencia de los exonerados.
