The Integrating Prevention into Connect Care (IPiC) for Health initiative will involve screening patients to identify
and address tobacco use, alcohol use, or physical inactivity during their
appointment or hospital admission, with the intention of creating more
opportunities to have conversations around behaviour change.
For patients screened as “medium” or “high risk,” they
will be offered information about how these risk factors may impact their
health, and/or a referral to community resources which will offer the patient
ongoing support if they feel ready to make changes. The screening results and whether referrals were made will be communicated with the patient's primary care provider on discharge summaries or consult summaries/notes on Netcare.
We are seeking your anonymous feedback on how we share your patient’s screening information with you to ensure it is helpful to you in supporting your patients around
their goals to address tobacco use, alcohol use, or physical inactivity. Specifically,
we would like to:
1. Know what information would be most helpful for you to receive about your patient
2. Learn your preferred method of receiving information about your patients (embedded
with discharge/summary notes or separate note/letter)
