Smart Tag Request form for Cash/Check Payments

 A $5.00 payment is required for a replacement badge. If you wish to purchase a lanyard or clear protector for the badge there will be an additional charge for each item. The option to request purchasing this item is located at the bottom of the page. It is recommended that the Heavy duty clear protector is purchased.



Please Note:

Any damage (punching holes, biting, bending, excessive heat etc.) will cause the badge to malfunction and will need to be replaced.
