  • Use this form to report event details for our records. If necessary, ask for a paper copy for keeping track of records at the event.
  • You are encouraged to take photos of your project/activity, but make sure you have signed photo/video releases for children and adults pictured in the photos. Add both the photos and release forms to the corresponding folders in the Shared Google Drive.
