As the legal parent or guardian of the child/ren identified below, I hereby consent to my child’s participation in the educational and recreational activity conducted by the Radha Krishna Temple, Allen, TX. 

No child will be allowed to check in without the parents consent and physical presence in the temple campus while the child is in the ongoing kids activity. As a requirement for emergency situations, one parent or guardian of the child should be present on the temple premises. 

The activities and games are for kids ages 4 and up.

In consideration of my child's™ participation in the Children's Program, I, the undersigned user, agree to release, indemnify, and hold harmless, on behalf of myself, my heirs, representatives, executors, administrators, and assigns HEREBY DO RELEASE, INDEMNIFY, AND HOLD HARMLESS Children's PROGRAM, its officers, agents, volunteers, employees, and sponsors Jagadguru Kripaluji Yog from any cause of action, claims, or demands of any nature whatsoever, including but not limited to, a claim of negligence, which I, my heirs, representatives, executors, administrators, and assigns may now have, or have in the future against Radha Krishna Temple, on account of personal injury, property damage, death, or accident of any kind, arising out of or in any way related to my use of the Radha Krishna Temple facility, whether that use is supervised or unsupervised, however the injury or damage is caused, including, but not limited to the negligence of Radha Krishna Temple, its officers, volunteers, agents, and employees.

I give permission for photographs taken at Radha Krishna Temple to be used for publicity purposes by Jagadguru Kripaluji Yog and/or Radha Krishna Temple of Dallas.
