APS (Ashlawn) Intent to Transfer or Withdrawal Notification Form 23-24
This form serves as intent notification that your child is leaving Ashlawn Elementary. You MUST complete the official notification document listed at the end of this google form . 

*Students withdrawing: Please be sure to turn in your child's ipad/ charger and library books before you leave Ashlawn!
*Student transferring within APS: Take your student iPad with you to your NEW APS school.

Please type your Parent first and last name here; this will count as your official signature that you understand you MUST submit the withdrawal notification form to officially withdraw from APS/Ashlawn.
