Dear parents,

Soundscapes is excited to announce a new fundraising opportunity for our program, which will create more educational activities for our students. Participation in this program is voluntary. Please sign below and return the form to Soundscapes if you are interested in your student participating.

The new program is called Sponsor a Child. With your permission, we would use photo/video of the students to encourage donors to sponsor at different levels. The student, if sponsored, would write a thank you note to his/her sponsor (stationery provided by Soundscapes). No other involvement of the student would be required. Sponsors will be given a list of dates where they can come to visit Soundscapes and see what the program is like first hand.

One hundred percent of the money raised through the Sponsor a Child program will go to benefit the Soundscapes program, which, in turn, directly benefits your child’s education at Soundscapes.

We look forward to this new program and the educational opportunities it will open up for us in the future. Please sign below if you’re interested in your child participating.

By participating in Soundscapes, I give permission and consent for my child to be photographed, interviewed, or video recorded during program activities. I further give permission and consent that any such media may be published via any media medium (i.e. print, broadcast, internet, or any other available medium) and used by Soundscapes and its agents, to illustrate and promote the program experience.
