This information will only be accessed by Unity of Fairfax for use in the event of an injury, sudden illness or other emergency or crisis situation while on our property.

In the event of an emergency, it is important to have accurate, up-to-date personal emergency contacts on-file; and so, we ask you to consider participating in this important initiative.
